I understand that this process is necessary for the safety and integrity of the organization and its participants. I authorize the release of any information that may be relevant to my suitability for volunteering, including but not limited to, criminal records, employment history, and personal references. I acknowledge that this information will be kept confidential and used solely for the purpose of evaluating my application. I understand that I must have a valid IVP clearance card before entering the school facility as a volunteer. I understand volunteers are required to sign acknowledgment forms for understanding school policies and confidentiality agreements.
